As far as I know, and as far as I've read, "Starting Machine Code" is not missing any material. On a personal level, I'm done reading "Practical MSX Machine Code", so the missing page is not a big deal for me, but someone in the future may still want it.
BTW, if there are any untranslated Japanese documentations out there that people want to see translated into English, I could offer to translate it, provided it be not too long... Anyway, it would be a way for me to contribute back, and probably a learning opportunity as well.

I updated the pdf on https://archive.org/details/StartingMachineCodeOnMsx with the PDF of page 4 that contains page 63 (yes, archive.org now has an edit function for uploaded items :)).
I don't mean "Starting Machine Code on MSX"! I mean "Practical MSX Machine Code Programming" which is missing page 63.
