Initially I thought that was a Rise Out hack, given the similary of the font (those 1+3px wide letters) and the HUD (MAN instead of the more traditional LIVES)...

...but after closer inspection, the font is not exactly the same and neither is the HUD (Rise Out has MEN, not MAN).
So it may be an hommage/clone of Rise Out?

The font may be a lead, at first it looked like Pony Canyon font to me, but it's not. If you find a game with the same font it may indicate the company behind the game...

Actually, I found a different or later version of this game from the same author. Now both versions are in the same place:

The funny thing is that both games work well on physical Yamaha MSXs, but don't work in the emulators. Once the game level is started, the lives count down to zero and the game ends. I will try to figure out why this happens. This is obviously abnormal because the emulator is supposed to be the accurate representation of the physical system.

Looks like the false sprite collision in the emulator is the reason for this strange behavior. And this happens not only in OpenMSX, but in other emulators as well. Probably it's some corner case. If someone from OpenMSX team could take a look, that would be great.

Very interesting! Can someone double check on a real machine?

Well, it works normally on my real turboR. Made an issue for openMSX: https://github.com/openMSX/openMSX/issues/1198

I fixed the games by modifying all unused sprites in VRAM:
